Zero Racial Disparities in Infant Mortality by 2033

FLOURISH has partnered with Legal Services of Eastern Missouri, St. Louis Children’s Hospital, Integrated Health Network, SSM Health Cardinal Glennon, Mercy Hospital and other community partners to collect data about issues related to Medicaid-provided transportation. FLOURISH hopes to collect 1,000 responses from April to June from its partners.

Medicaid provides patients transportation to appointments with a health care provider or for a health care service, which is covered by MO HealthNet or MO HealthNet Managed Care.

Do you have a patient with a complaint about their non-emergency medical transportation? Let us know.

  1. Ask patients about their experience with non-emergency medical transportation (their ride to the office).
  2. Fill out a survey for them online or on paper.

Paper surveys can be submitted to Legal Services of Eastern Missouri:

By Email: ajschneider@lsem.org

By Fax: (314) 534-1028

By Mail: 4232 Forest Park Avenue, St. Louis, MO 63108
